The Care Coordination for Children (CC4C) Program is a service that focuses on children ages 0-5 and their families in Sampson County. The goal of CC4C is to improve the care of children by linking families to services that will meet their specific needs. The program is offered by staff located at the Sampson County Health Department.

CC4C is a program offered at no charge for children birth to 5 years of age who have long term medical conditions; are dealing with challenges with their environment that may increase their stress levels; have risk factors that can affect physical, developmental or mental well-being; and/or are referred by their provider.

Once a referral is received, a CC4C Care Manager is assigned to the family to assess the needs of child and family. The plan of care and goals are developed together by the CC4C Care Manger and the family. The CC4C Care Manager work with the family through home visits, phone calls, letters, provider visits, and other types of contacts. The frequency of the contacts usually depends on the needs of the family. The CC4C Program works to build strong family and personal relationships with everyone involved in the care of the child.

Sampson County Health Department offers the Child Health Well Visit Clinic that provides well child care visits for children ages 0-21 years old. North Carolina has minimum requirements that must be met for entry into pre-kindergarten and kindergarten. Generally a child must have had a recent physical exam and be up to date on immunizations. North Carolina Law and the Sampson County and Clinton City Schools require specific paperwork to be completed to show that these requirements have been met. Individual daycares and Head Start Programs can also require physical examinations and immunizations for entry into their programs. The Sampson County Health Department is able to provide the physical exams and immunizations for children entering into Daycare, Preschool, Head Start, More at Four, Kindergarten and school.

The Sampson County Health Department Child Health Clinic utilizes the Bright Futures Program. The goals of the program are: to ensure the health and well-being of the children of Sampson County; and provide the possible early detection of medical or other factors that can impact the child’s future by incorporating the Bright Futures principles, strategies, and tools. Bright Futures principles acknowledge: the value of each child; the importance of family; the connection to the community; and that children with special health care needs are children first. These principles are used at the Sampson County Health Department Child Health Clinic to assist in delivering, and supporting the highest quality health care for children and their families.

The Bright Futures Program Clinic begins the visit with a questionnaire, which a parent or patient completes at the beginning of the visit. A nurse reviews the information and provides the appropriate age-specific screening that incorporates the information gathered from the questionnaire. This is followed by the physical exam. The Bright Futures Program also provides follow-up with age specific tasks that are assessed with each subsequent preventive visit and parent/child anticipatory guidance material.

The Child Health Clinic is held each week by appointment, but walk-ins may be accepted. Parents may schedule appointments by calling the Sampson County Health Department at 910-592-1131, ext 4001. Medicaid, Health Choice, and private insurance are accepted.
